Mental Health and Wellbeing Support

The Mental Health and Wellbeing Practitioner will help you by listening to whatever struggles you have in your life that are having an impact on your health. They will work with you to build resilience so you can independently build your best life.

  • Access - Open, no GP referral required, 18 and over
  • Speed - medium (days/weeks)

  • required insight & autonomy - medium (need own motivation to change)

This service will support you with things that you are struggling with that are impacting on your health, these may be: stress, low mood, bereavement, loneliness, debt, anxiety, identity issues, addictions, chronic health conditions.

Access is open, you do not need to be referred by a GP but you do need to be over 18. Ask any member of staff at your GP practice to ask the practitioner to get in touch and they will contact you to arrange an appointment.

You will be seen on a one to one basis either in person or on a video or telephone call to help you to discover which pathway to your best life would work for you. Usually this would be for one session which could last up to an hour.

During your appointment the practitioner will listen to your needs and help you to find people and places that might help, will assist you to learn techniques to combat distress and feelings that are uncomfortable and refer you for ongoing care if needed.
